Background

From time to time, Access Fertility Limited (“we”, “us” or “the Company”) may be required by law to provide to you certain written documents, agreements, notices, disclosures, authorisations, acknowledgements and other communications (“Communications”).

By clicking “I Agree”, you confirm and agree as follows:

1. Scope of Consent
   I consent to receive all Communications relating to my relationship with the Company, now and in the future, in electronic form through the Access Fertility Patient Portal (“AFPP”), email or other electronic means.  This consent applies to all current and future Communications, until I withdraw it as set out below.
2. Electronic Signature Agreement
   I agree that my electronic signature (including typing my name, clicking “I Agree”, or using a digital signing tool) is the legal equivalent of my handwritten signature and has the same legal effect and validity for all Communications I sign or approve electronically until I withdraw my consent.
   
   
3. Right to Paper Copies
   I may request paper copies of any Communication at any time by emailing support@accessfertility.co.uk and providing my name, postal address, email address, telephone number and Patient Reference Number.  The Company may charge a reasonable administration fee (including printing and postage costs) for paper copies.
   
   
4. Withdrawing Consent
   I may withdraw my consent at any time by emailing support@accessfertility.co.uk and stating my name, postal address, email address, telephone number and Patient Reference Number.  We will act upon your request within 30 days.   

5. Consequences of withdrawing consent
   Withdrawal will not affect the legal validity of any Communications signed electronically before withdrawal but may delay the provision of services.  Withdrawing your consent to receive Communications electronically from us means that you will no longer be able to use the AFPP to receive or sign Communications electronically.  If you elect to receive Communications only in paper format, it will slow the speed at which we can complete certain steps in transactions with you and delivering services to you because we will need first to send the required Communications to you in paper format, and then wait until we receive back from you your acknowledgement of your receipt of such paper Communications.
6. Keeping Your Contact Information Up to Date
   I will notify the Company promptly of any changes to my contact information by emailing support@accessfertility.co.uk with my old contact details, new contact details, full name, and Patient Reference Number.
7. System Requirements
   To access and retain Communications, I must have:

• • An internet-enabled device
  • A valid email account
  • A current web browser with security updates
  • The ability to download and store documents (e.g. PDF viewer)

8. Acknowledgement of access to consent
   By clicking “I Agree”, I confirm that I can access and read this consent and that I can save or print it for my records .
